In order to comply with Federal Communication Commission (FCC) regulations, telephone companies are required to safeguard and protect customer’s personal information, otherwise known as Customer Proprietary Network Information (CPNI). You must complete an authorization form to be retained in your file. This form allows us to protect your information and will allow only the individuals that you have chosen to have access to your account.
The authorization and password form must be filled out and returned in order to access certain information regarding your account.
If a password is not set up, we will not be able to give out certain information by phone and valid photo ID will be required to give this information out in person.
The authorization form can only be completed and signed by the account holder, as we are required to verify all signatures. If we are unable to verify your signature, you will receive written notification that the CPNI Change Form has not been accepted.
Once your signature has been verified, you will receive written notification that a change has been made to your account. The letter will not contain the password or other security information.
If you receive written notification of a change to your account and you did not authorize the change, please contact your local office immediately and report the problem. A Customer Service Representative will work with you to secure your account and all personal information.
Some customers may have multiple service accounts with Inland Networks. If you would like to have the password, Security Questions and Authorized Users established for multiple accounts, you will have the opportunity to do so. Inland Networks encourages you to secure all of your accounts for your protection.
If you have already secured your Inland accounts, thank you. You do not need to complete this form. To verify if you have completed CPNI security in the past, please contact your local service office.
